Out of 4 people (including myself) frequently playing the Wii and 3 others that have played a little bit, none have lost grip of the Wiimote and there have been no Wii related injuries. I've had some close calls standing near the vicinity of someone playing tennis so I backed off a bit more, the same person also puts a lot of effort into swinging (especially in baseball) and I'm scared that it'll go flying. In my own experience, I can't imagine myself losing grip even when I'm putting all my effort into swinging, the only injury I can imagine me causing is someone standing too close to me and getting hit.

I have never lost grip but Iv'e changed hands while the strap was still on my right hand and it slips but it always dangles and never hits the ground.

I don't know how it happend..but it did. The Wii - Mote slipped out of my hand in bowling. ( I diden't have a good grip with the mote i remember) I had my wrist strap on..but it swun up via the wrist strap and slapped me just under the eye.
Two new theorys. 1. The wrist strap is also dangerous. and 2. Quote:

A friend of mine came over today and played Wii. He had a difficult start figuring out the controls, especially for bowling. I forget exactly what he was doing, I think it was bowling, and I'm sure I told him to put the wrist strap on but I forgot to tell him to tighten it, so before long the Wiimote went flying behind him (he bowled with a sideways swing). Fortunately it hit (or came really close to) my brother instead of me, no injuries (drat). I told him how to properly tighten it and since he kept taking drinks and visiting the kitchen he put it down a few times and before long I caught him with the strap completely off just as we were about to start tennis. He also had a hard time with ExciteTruck and quit in the middle of the first race against me. Despite his troubles, he has a better opinion of the Wii than he had before.
